When a unit becomes available, you will be contacted by the Administrator. Our nurse will schedule an in-home visit and complete an assessment of your needs and have you sign a medical release of information form.
What you will need before you move in:
- Your Doctor will be asked to provide:
- History and Physical done within the past six (6) months.
- Doctor’s Orders, Medication Orders, and Diet.
- Doctor’s Orders for over-the-counter medications (Tylenol, aspirin, laxatives, Tums, etc.)
We need to have doctor’s approval for these items to ensure there are no drug interaction with your other prescribed medications.
- Doctor’s orders for Vitamins and/or dietary supplements.
- Any treatments needed, such as oxygen use, Physical Therapy, etc.
If you do not have a preference, we can recommend a local pharmacy.

Outside Services (paid by Resident):
- Telephone: Residents may use our telephone to make local call;
however, if you wish to have a private telephone in your apartment, you may contact the telephone company for arrangements.
- Notify the Newspaper of your choice if you wish to have a paper delivered to your room.
- Cable Television: Notify the Cable Company if you wish to have the cable television in your room activated.
Hook-ups are provided in each apartment.
Notify the Post Office of a Change of Address (we have these forms)

Legal Papers:
- Living Will: We will need a copy of your Advanced Directives if you have one.
If you do not have one and wish to know more about it, we can advise you of a contact source.
- Power of Attorney: If applicable, we will need a copy of the document.
- CPR (Cardio Pulmonary Resuscitation): We will provide this form for you to sign upon admission.
We accept Medicaid Assistance. You will need to contact your Eligibility Office and have a regional Medicaid Unit Nurse assess your needs before moving in.
